It's done in Adobe Illustrator.
I use photographs as a reference, to check with the highlights and shading.
First I use the pen tool to draw the outline.
Then comes the colouring part. The red colouring on the cheeks is actually a different piece on which effects like feather and transparency have been used. Then I place the piece on the cheeks and forehead... likewise for the shading/shadow and highlighted areas :)
